About Us

We are Chester South Primary Care Network

Chester South PCN is a collaboration between four general practices across South Chester with a combined population of just over 34,000 local people.

Although our practices still operate independently, we are now providing certain services together. These include a Social Prescribing Service offered by our Well-Being Co-ordinator and Social Prescribing Link Worker, Direct Access Physio Service, and Direct Access Low Level Mental Health Service.

We will be working in partnership across the health and care sector to offer more of the services needed by our Chester South Primary Care Network registered population.

It's a new name for us, but it's really about providing you with the right services, by the right people, at the right time for many more years to come.

What are we working on?

The Chester South PCN is already developing new approaches and offering new services based on what our local population need. Longer term, this will help us work together to recruit staff that can compliment the needs of our patients and offer a clearly defined range of services to patients across our practices.

What is a Primary Care Network?

Primary Care Networks (PCNs) have been created to provide proactive, coordinated care to their local populations, in different ways to match different people's needs, with a strong focus on prevention, population health and personalised care. This means supporting patients to make informed decisions about their own health and care and connecting them to a wide range of statutory and voluntary services to ensure they can access the care they need first time.

How will the PCN impact our patients?

The benefits of working as a PCN include:

  • over time patients will see a range of new clinical services available in their local GP surgery
  • patients may be offered an appointment or be seen by a range of different health professionals which the GP feels are best able to meet your needs
  • patients may also see a greater focus on prevention with more attention on community-based activities that will help people live healthier lives
  • some longer GP consultations - up to 20 minutes - if the GP thinks you need one
  • same-day emergency appointments or phone consultations when you need them, not next week
  • different types of appointments - telephone, online, and digital consultations where a tap and a click could save you a trip
